We’ve been waiting for this reminder to arrive from the New York State Department of Health (NYSDOH) because there have been several high-level citations related to negative outcomes from the use of side rails in New York nursing homes recently. During CMSCG mock surveys and on-site visits to clients, this is one of the areas we review during environmental rounds. In addition to reviewing the actual bed and side rail in use, assessing the accuracy and comprehensiveness of the associated clinical risk assessment and care planning documentation if there are potential issues identified because there is so much risk associated with side rail use. It’s important to keep on top of this high-risk area, and NYSDOH obviously agrees, as it indicated in the June 13, 2025 DAL, “Entrapment Risk with Bed Systems and Components.”
The DAL (DNH-25-06) reminds nursing homes that they should ensure staff are “especially diligent” when evaluating the risk of entrapment associated with the use of bed rails. The DAL also refers to F700 Bed Rails, including the regulatory requirement that appropriate alternatives are used prior to installing bed/siderails. If they are used, the facility is responsible for ensuring the correct installation, use and maintenance of the rails. Don’t forget about the regulatory expectation regarding routine inspections.
