- Entrance Conference Worksheet (January 2022 – Valid for non-injunction states)
- CMS-20054 Infection Prevention Control and Immunization January 2022 (Valid for non-injunction states – the date of the form for the states with delays is still 9/2021)
- LTCSP Mapping Document_Streamlined
- LTCSP Procedure Guide (Effective 1/27/2022)

- FIC Survey Entrance Conference Worksheet (dates for use are based on the three recent QSO Memos issued by CMS regarding the staff vaccination mandates, with January 27, 2022 being the start date of the first group)
