INTERACT (Interventions to Reduce Acute Care Transfers) tools have been updated to version 4.0. INTERACT is a quality improvement initiative that is geared towards reducing hospital transfers, and includes communication tools, care paths/clinical tools and advance care planning tools. The new tools include the following changes:
- Revision to the SBAR Communication Form and Progress Note
- Criteria updated for Decision Support Tools (Change in Condition File Cards and Care Paths)
- Revision to Stop and Watch tool
- Revision of Advance Care Planning Tool
- Update of Quality Improvement Tool to include changes in condition
- Update to the Implementation Guide
