483.21 — Comprehensive Resident Centered Care Plan (F655–F659)
The Comprehensive Resident Centered Care Plan regulatory group establishes CMS requirements for developing and implementing person-centered care plans for every resident. These regulations govern the baseline care plan developed within 48 hours of admission, the comprehensive care plan developed following the full MDS assessment, care plan timing and revision requirements, professional standards for services provided, and the qualifications of persons delivering those services. Care planning deficiencies consistently rank among the most cited nationally — F656 ranks in the national top 5.

Comprehensive Care Planning — F-Tag Analysis
Baseline Care Plan
Requirements for developing and implementing a Baseline Care Plan within 48 hours of admission — including the minimum required elements, summary requirements for residents and families, and the consequences of failing to identify resident needs at admission. Four IJ-level citation examples reviewed.
Develop/Implement Comprehensive Care Plans (2-Part Series)
Requirements for person-centered, comprehensive care plans with measurable objectives and timeframes — covering what must be included, how to write measurable goals, cultural and trauma-informed care planning, resident refusal documentation, and the distinction between generic templates and true person-centered care.
Care Plan Timing and Revision
Requirements for completing the comprehensive care plan within 7 days of the MDS assessment completion date and revising the care plan whenever a resident's status changes or new needs are identified.
Services Provided Meet Professional Standards (2-Part Series)
Requirements for services to meet professional standards of quality — significantly revised in April 2025 to address mental health diagnosis documentation, particularly Schizophrenia. Covers insufficient documentation examples, DSM-5 diagnostic criteria, and what surveyors are directed to review when evaluating diagnosis validity.
Qualified Persons
The care plan only works if the people implementing it are actually qualified to do so. F659 is one of those tags that doesn't get a lot of attention — until a surveyor starts asking questions about who is providing a specific service and whether that person is qualified to provide it.
Need Support Strengthening Care Planning Compliance?
Care planning deficiencies are among the most commonly cited in nursing homes nationally. CMSCG's consultants help facilities develop person-centered care plans, strengthen IDT processes, and identify care planning vulnerabilities before a surveyor does.
