A new study published in the Journal of the American Geriatrics Society highlights a new care model that is being sponsored by an award from CMS called OPTIMISTIC. OPTIMISTIC, which stands for Optimizing Patient Transfers, Impacting Medical Quality and Improving Symptoms: Transforming Institutional Care is a program developed by Indiana University and the Regenstrief Institute that seeks to improve care and communication in long term care facilities so that issues can be addressed before a resident requires a hospital transfer.
