OPTIMISTIC, a long term care model that stands for “Optimizing Patient Transfers, Impacting Medical Quality and Improving Symptoms: Transforming Institutional Care,” has received a grant from the Hartford Foundation. This grant money will help with evaluating the program, developing training materials and will provide the support to help scale the model to a national model.
Don’t want to wait until OPTIMISTIC is rolled out on a national basis to start leveraging the principles of improved care quality, reducing hospitalizations and providing additional access to palliative care? Check out CMS Compliance Group’s article from the CMSCG Bulletin, “Thinking OPTIMISTICally.”
