OPTIMISTIC, a long term care model that stands for “Optimizing Patient Transfers, Impacting Medical Quality and Improving Symptoms: Transforming Institutional Care,” has received a grant from the Hartford Foundation. This grant money will help with evaluating the program, developing training materials and will provide the support to help scale the model to a national model.
